"Advanced" should be selected for participants in 7th grade or higher with prior dance experience.
By submitting this form, I am agreeing to participate in the CSHS Silverado Dance Clinic. I understand that my child will be participating in dance/aerobic activity for an extended amount of time with allowed breaks for rest and hydration. I will not hold Clear Springs High School, CSHS Administrations, Mrs. Kelsey Collins, the Silverado Booster Club, or CCISD responsible should any injury occur while attending the CSHS Silverado Dance Clinic.
