By submitting this form, I am agreeing to participate in the CSHS Silverado Dance Clinic. I understand that my child will be participating in dance/aerobic activity for an extended amount of time with allowed breaks for rest and hydration. I will not hold Clear Springs High School, CSHS Administrations, Mrs. Kelsey Collins, Ms. Madison Martin, the Silverado Booster Club, or CCISD responsible should any injury occur while attending the CSHS Silverado Dance Clinic. This event is a fundraiser; therefore, once your registration has been recieved, we are unable to offer refunds. Your support and understanding is appreciated.
